The Puerto Rico Health Declaration is a recently launched travel document that all travelers visiting this country must display to be allowed to enter. It is a declaration that works to verify the applicant’s health condition.

WHO CAN ENTER PUERTO RICO WITH THIS TRAVEL DOCUMENT?
All adult travelers, including Puerto Ricans going back to the country, must apply for this document before arrival.
GET THE PUERTO RICO HEALTH DECLARATION
To get the Puerto Rico Travel Declaration Form, you’ll have to provide information about yourself and your flight. If you don’t own a flight ticket, you won’t be able to complete the form just yet.
You will also be asked to submit a negative molecular COVID-19 test result (nasal or throat swab) not older than 3 days. If you’re not able to show proof of testing, you will have to undergo quarantine. The PCR test results must be uploaded to the Puerto Rico Health Department’s online portal. Please be aware that in case you have a COVID vaccination certificate to enter the country, you’ll be able to travel without so many limitations like quarantine.

Selfie holding the physical credit card used for the transaction is need to verify payment. Only the last 4 digits of the credit card must be visible in the picture. Only one image is needed per order. (if applicable)
All travelers must present a Negative PCR test done within 72 prior departing Puerto Rico. However, vaccinated travelers from U.S Territories (United States, American Samoa, Guam, Northern Mariana Islands, US Virgin Islands, United States Minor Outlying Islands) don't need to get tested for COVID-19 before visiting.
The Puerto Rico Health Declaration is part of the required document you will need to enter Puerto Rico, it DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for your Visa when you enter Puerto Rico.
Please be advised that Puerto Rico now requires a COVID vaccination certificate to enter the country. If you do not have one, you are still subject to restrictions upon arrival, such as taking a PCR test and mandatory quarantine. With a COVID-19 vaccination certificate, you'll be able to travel in and outside the country freely without limitations.
