The Bangladesh Health Declaration Form is a mandatory requirement for all passengers entering through ground crossings, by seaport, or by airport. The Ministry of Health & Family Welfare, Government of the People’s Republic of Bangladesh issued its necessity as part of a health screening process at the port of entry.

It is a basic PDF form to advise the Government of your current health status. All passengers must complete this form before check in.

It does not replace a visa or any other required travel authorization.
