Regardless of nationality, age, or any other considerations, all visitors arriving in Ecuador must fill out a Traveler Health Declaration (COVID-19) to be authorized to enter the country.

The purpose of this travel form is to guarantee the protection of residents and non-residents from the time they board the plane to the time they enter the country.

Through this Traveler Health Declaration, passengers must truthfully assert their state of health along with their flight details, place of accommodation for the next 21 days, and emergency contact information. This document does not replace a visa in any way.

You must fill out the Ecuador Traveler Health Declaration at least 24 hours prior to your departure date. Be advised that if you don’t have a flight ticket, you will not be able to fill in the form yet.
