OBTAIN THE SPAIN TRAVEL HEALTH CONTROL FORM
To apply for this document, you’ll have to provide information about yourself, your health status, and your intended stay in Spain. In addition, you are required to have a confirmed flight ticket in order to complete the form.

  • You can apply anytime, but the Form will be sent to you 2 days before your arrival in Spain, since the health questions are available when there are 48 hours left for your arrival.

  • At the airport, you must show the QR Code that we will bring you, in order to pass the health check established by the Spanish Health Authorities (SpTH App).

  • It is highly recommended that you keep your Form and the associated QR Code with you either as a soft copy (smartphone, tablet, etc.) or as a hard copy in case of any failure in the system.
