The New York Traveler Health Form is a recently launched travel document that all travelers who wish to visit New York must fill out. This Health Form was created to help the US authorities to have better control of passengers who may have been exposed to COVID-19.

WHO CAN ENTER NEW YORK WITH THIS DOCUMENT?
Anyone entering New York from a US state that has a notable degree of community-wide spread of COVID-19 must fill out this document. Foreign travelers must also apply for this document, regardless of the means of transportation to New York (plane, car, or train).
Travelers from states that are contiguous with New York are exempt. However, covered travelers must continue to fill out the Traveler Health Form. Essential workers don’t have to apply for this form.
Please take into consideration that the United States has restricted the entry of all foreign travelers who have passed through or have been in the following countries in the last 14 days: China, Iran, Austria, Belgium, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Slovakia, Slovenia, Spain, Sweden, Switzerland, Monaco, San Marino, Vatican City, the UK, Ireland, and Brazil. The exception to this rule includes those travelers with diplomatic visas and those with family members who are US citizens or permanent residents.
OBTAIN THE NEW YORK TRAVELER HEALTH FORM
To get the New York Traveler Health Form, you’ll have to offer some information about your flight and your health status so that you can confirm that you have no signs of COVID-19.
Keep in mind that all travelers (including US citizens, permanent residents, and transit travelers) wishing to enter the US must show a negative COVID-19 viral or antigen test result issued 72 hours before departure. Additionally, travelers who were out of New York for more than 24 hours must quarantine for three days upon their arrival.
Travelers who were out-of-state for less than 24 hours don’t have to get a COVID-19 test, and they don’t have to quarantine.

Important Instructions

Travelers from border states (NJ, CT, PA, MA, VT) are not required to quarantine or test. Non-essential travel is discouraged.

You can apply anytime, but remember we can only process your application with a max of 24 hours prior arrival.

Essential workers from other states and countries are not required to quarantine. However, NYS essential workers and long-term essential workers are required to get tested 4 days after their arrival to NYS.

Travelers passing through another state or country for less than 24 hours, other than in the course of travel, are not required to quarantine. However, the traveler must take a COVID-19 test 4 days after their arrival in New York State.

Travelers who had a COVID-19 test prior to coming to NYS must take a second COVID-19 test on day 4 after arrival, and are required to quarantine for a minimum of 3 days upon arrival, and are no longer required to quarantine upon receiving a negative result from the second test (the test taken in New York).

All other travelers that are asymptomatic are required to quarantine for 10 days if they do not test prior to departure and on day 4 after arrival.

Travelers must continue to monitor symptoms through day 14. If any symptoms develop, travelers should immediately self-isolate and contact their health care provider or local health authority to report this change and determine if they should seek testing.

Nevertheless, anyone entering NYS from a non-bordering state or traveling internationally from a country indicated as a CDC level 2 or higher COVID-19 travel health warning must apply for the New York Traveler Health Form, with no exception.
Yes, all travelers who have to fill out the form must also obtain a test within three days of departure, before arrival in New York. This rule is the same for travelers entering the US.
For children or people with incapacities to fill in the form on their own, it will be the duty of the father, mother, or person in charge to complete the form on their behalf.

Travelers who were out of New York for more than 24 hours or who are coming from a CDC Level 2 and higher Health Notice country must quarantine for three days once they arrive in New York. On the fourth day of the quarantine, the traveler must take another COVID test. If both tests are negative, the traveler may exit quarantine early upon receipt of the second negative test.
Keep in mind that all travelers returning to the US are advised to stay home as much as possible.
