The Comoros Health Form is a mandatory travel document designed to stop the spread of the coronavirus among nationals and foreigners. All travelers must fill it out and report their medical condition before arriving in the country. It does not replace a visa or any other travel authorization.

WHO CAN ENTER COMOROS WITH THIS DOCUMENT
All arriving passengers must complete the Comoros Health Form before they enter the country. Without this document access to Comoros will be denied.

All visitors wishing to obtain the Comoros Health Form must fill out the application form and provide essential information about themselves, their trip, and their medical status. It is required to choose the processing time for the document and attached other required paperwork, such as a scanned passport.

  • All travelers entering Comoros, regardless of their nationality, age or any other consideration, must have registered in the Comoros Health Form before arriving.

  • It DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for your Visa when you enter the country. Keep in mind some nationalities will require a paper visa. If that is your case, we suggest you contact your local embassy.

  • A negative Coronavirus (COVID-19) PCR test result taken 72 hours before departure is needed. Fully vaccinated travelers are exempted from this requirement.
