US citizens need a visa to visit India. United States passport holders are eligible to apply for Indian eVisas for short-term travel such as tourism, business, or medical treatment. In addition, they must complete the e-Arrival Card, a digital immigration and customs form.

India Medical eVisa: an electronic visa for US citizens traveling to India for medical treatment at recognized healthcare facilities.
India e-Arrival Card: A mandatory digital form that must be completed online within 72 hours before arrival. After submission, travelers receive a QR code for inspection at immigration.

Passport-style photo (if required)
Confirmed onward or return travel ticket
Proof of accommodation and travel itinerary
