Anyone who wishes to enter this country must fill out the Peru Affidavit 72 hours before their flight. All nationalities can apply.

The Peru Travelers Health Affidavit is currently aimed at all nationalities, including the same Peruvian citizens who have visited other countries and are returning to Peru.
Unvaccinated travelers need to present a COVID-19 PCR test result taken no more than 72 hours before departure. Travelers under 12 years of age only need to show a medical certificate of good health.
The Peru Travelers Health Affidavit is a Single Entry document and expires 1 days after arrival allowing a max stay of 90 days Per Entry.
Upon arrival in Peru, you need to present your biometric passport along with your Travelers Health Affidavit.
