Why Register with the Embassy
Travel Registration is a service provided by the government. This service allows you to record information about your upcoming trip abroad to the Department of State so it can be used to assist you in case of an emergency. ZAMBIA HEALTH DECLARATION FORM
- Health Declaration Form: A traveler health questionnaire that must be filled in before traveling to Zambia.
- Validity: Valid on entry to Zambia
- No. of entries: A single entry document

- HD Eligibility: Everyone who wishes to travel to Zambia must fill out the Health Declaration Form

What can I do if my Zambia Health Declaration application has been canceled or rejected?
