The Namibia Health Questionnaire is a mandatory travel document that all tourists wishing to visit Namibia must submit before their departure. It is part of the new health measures implemented by the government. This document does not replace a visa.

WHO CAN ENTER NAMIBIA WITH THIS DOCUMENT
To go to Namibia, all visitors must fill in the Namibia Health Questionnaire to enter the country prior to their arrival.

All nationalities can apply.

OBTAIN THE NAMIBIA HEALTH QUESTIONNAIRE
If you want the Namibia Health Questionnaire, you will have to provide specific information about your health status in the 14 days before your trip, where you will stay and which places in the country you will visit.

  • All travelers entering, regardless of their nationality, age or any other consideration must carry their Namibia Health Questionnaire (COVID-19) printed and signed.

  • The Namibia Health Questionnaire (COVID-19) is a Single Entry document. It DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for a Visa when you enter the country. Keep in mind some nationalities will require a paper visa. If that is your case, we suggest you contact your local embassy.

  • At the airport, you must show printed the Namibia Health Questionnaire (COVID-19) that we will send you through email, in order to pass the health check established by the Health Authorities.

  • After landing, temperature scanning will take place, and passengers showing any symptoms will be tested again for Covid-19 at the airport.

You could change the information if the Namibia Health Questionnaire was not processed yet; so, in this case, you would be able to upload the correct materials for the health code. However, once the document is processed, you won’t be able to edit it. You will then have to wait for the rejection and reapply.

If your application was declined, you are eligible for a full refund of the iVisa processing fee. However, we do not refund the government fees.

You will receive the Namibia Health Questionnaire via email once the application is accepted. In addition, you will receive an email notification if your application has any problems.
