Additional requirements for non-epassport renewal (brown, green, machine readable, or older passports): The Philippine passport holder must present a valid ID as well as a photocopy.

Requirements for a minor applicant

Confirmed online appointment.
Accomplished application form.
Current ePassport with a photocopy of the data page.
Marriage certificate if only one parent is accompanying the child. If parents are unmarried, a Special Power of Attorney (SPA) executed by the mother is required if she isn’t accompanying the child.
Original and photocopy of proof of filiation or guardianship.
Passport or valid government-issued ID of either parent with one photocopy.
