Pakistan Health Declaration form is a document that ensures the lawful entry of foreign citizens to the country, regardless of their nationality. Foreigners are required to follow the conditions laid down in the same. Furthermore, foreigners are not allowed to reside in Pakistan without legal documentation.

The Pakistan Health Declaration form is a general Immigration form that can be used for all non-immigrant and non-national visa holders. It has been in use since the country was established. This document has been available to all foreigners coming to Pakistan since 1993. Foreigners do not need any special clearance from Pakistani government agencies to obtain the form.

To apply for the health declaration form, the foreigners have to go to the Pakistan embassy or consulate. These agencies provide this form to all applicants as soon as they apply for their visa or passport. There are a number of agencies that provide a visa application form. They even provide the software to convert the form into PDF format for those who may not be familiar with the format.

The name of the applicant should be mentioned as the applicant’s name in the form. The full legal name should be written as the applicant’s name. The name of the applicant must also appear on the form in full. The Social Security Number or SSN of the applicant is to be mentioned in the social security number field. Any other fields that the applicant wants to put in the form should be indicated in the same way.

The date of birth of the applicant is also marked in the section of the form. The age of the applicant should be in the legal age bracket for Pakistan and must be entered as the legal age. The gender of the applicant is to be indicated in the section of the form. This is the only field in the form that the gender of the applicant is required to be entered.

In the details of the place of stay and mode of transportation, the applicant must state the name of the place in the place of stay field. This should be put in full. The first section of the form requires that the applicant must indicate whether the applicant is a foreign citizen or a national of Pakistan. If the applicant is a national of Pakistan, then the applicant has to put the full legal name of the applicant in the name section. If the applicant is a foreign citizen, then the applicant has to put his/her passport number.

The second section of the form requires that the applicant’s place of employment should be indicated in the employment section. The place of residence should be indicated in the place of residence section.

The last section of the form requires that the applicant is not a citizen of Pakistan or a foreign citizen. The applicant must show that he/she is not a Pakistani citizen or a foreign citizen. The applicant has to declare his/her date of birth in the birth date section.

All the other details of the applicant’s entry should be listed in the section. The applicant has to write the address of the applicant in the address section. The name of the applicant’s employer is to be mentioned in the employer’s section.

All these details are to be indicated in the name section of the form so that the applicant may be able to prove that he/she is a foreign citizen or a Pakistani citizen. The foreign citizenship cannot be confirmed by the application of other documents, so it is important that the application must be accompanied by the necessary documents. The application should not be rejected because the foreign citizenship is not confirmed.

All visas grant travelers the permission to travel to a country's port of entry. Whether you are granted entry or not will be at the discretion of the Immigration Officer at the entry point.

We know that our applicants can come across unexpected changes in their travel plans that may prevent them from traveling and therefore getting their visa. This is why we understand you may want to request a cancellation of your visa application and a credit for the amount paid. However, we will only approve the refund/credit if it follows our Refund policy guideline:

  • If your application has not been reviewed by our team, nor submitted to the government for approval, then you will receive a credit for the full amount of the iVisa processing fee and government fee(visa cost).
  • If your application has been reviewed by our team but has not yet been submitted to the government for approval, you will receive a credit for the full amount of the government fee (visa cost).
  • If your application has been submitted to the government for approval or a visa is already issued, unfortunately, no credit is available.

Our policy also applies to these circumstances:

  • If we make a mistake on your application, a 100% refund of the iVisa processing fee will be issued and we will process your visa again at no cost.
  • If you purchase rush or super rush processing additional fee and we do not deliver the travel document within the agreed timeframe, you will receive a credit for the iVisa rush or super rush processing fee.
  • If you miss your trip because iVisa does not deliver the travel document within the agreed timeframe, you will receive a full cash refund for the government fee and iVisa processing fee.
    • The exception to the clause above is if the government is the cause of the delay or your arrival date was sooner than our super rush processing time. In such case, you will receive a credit for the iVisa processing fee.

If for any reason, you are not satisfied with our services, please feel free to contact us. Under reasonable circumstances, we will grant a credit of the iVisa processing fee to be used for a future order.

When the visa has to be shipped to you, the processing times do not include this shipping time. It is from when we receive your application to obtaining the approval. Most visas (e-Visas) are sent via email upon approval so shipping times will not apply.
