DOCUMENT OVERVIEW The Belize Health Declaration Form is a new travel document that all travelers must complete before traveling to Belize. This document will assist all travelers in submitting the requisite travel and health information to facilitate travel into Belize. It also has a geo-tracking functionality for all travelers during their stay in Belize to assist local public health officials in monitoring and contact tracing.

WHO CAN ENTER BELIZE WITH THIS DOCUMENT? All visitors must apply for this document, including citizens from Belize that are returning to the country.
GET THE BELIZE HEALTH DECLARATION FORM To get the Belize Health Declaration Form, you’ll have to offer some valid information about your health status and your travel to Belize. You’ll also have to provide a negative COVID-19 RT-PCR test performed within 96 hours before you arrive in Belize or take a COVID-19 test upon arrival. Keep in mind that depending on the results of the health exam, you may be randomly selected for a second test. Covid vaccine certificate holders won't need to present a negative PCR test nor to follow a quarantine.

DISPLAY THE FORM Once you complete the application request, we’ll send you the Belize Health Declaration Form to your email before your trip. This will generate a QR Code and TRIP Number, which must be presented upon arrival.
Although it is not mandatory, we recommend you to print out the Belize Health Declaration Form if needed so that you can present it to the immigration officers once you arrive in Belize.

Passengers must complete an online health declaration form within 72 hours before arriving in Belize.

The application will generate a QR code and a trip number (TRIP) that must be presented upon arrival.

Passengers must provide a negative COVID-19 RT-PCR test performed within 96 hours prior to arrival in Belize or take a COVID-19 test upon arrival in Belize. Covid vaccine certificate holders won't need to present a negative PCR test nor to follow a quarantine.

A negative test will speed up the arrival process. However, depending on the results of the health exam, the passenger may be randomly selected for a second test.
