You need to fill in a separate form for children (including newborn babies). If you have more than one child traveling with you, then you have to fill in and apply using a separate form for each child, and carry the visa approval along with a valid child's passport.

In such cases, minors applying must be in possession of their own separate passport.

Young children above the age of 2 years and below the age of 12 traveling without a parent or legal guardian are considered unaccompanied minors. Most airlines provide a service to assist these children from their point of departure to their final destination.
