You can obtain the Nigeria Health Declaration Form online. Simply apply online with us by offering some personal details like full name and passport ID number.

To enter the country, you must have a negative COVID-19 PCR test result taken not more than 96 hours before boarding.

DISPLAY THE FORM
Once you finish the application process, you’ll receive the Nigeria Health Declaration Form with the QR Code via email inbox. Although you can easily keep this document on your cell phone, it’s still advisable to take a hard copy of the COVID-19 PCR test for presentation at the airport.

You will have to undergo temperature screening upon arrival, and authorities will ask you about COVID-19 symptoms. If your temperature is more than 38°C and you have other COVID-19 related signs and symptoms, you shall not be allowed to board the flight.

Important Instructions
  • Nigeria Health Declaration Form is part of the required documents you will need to enter Nigeria. It DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for your Visa when you enter Nigeria. Keep in mind some nationalities will require a paper visa. If that is your case, we suggest you contact your local embassy.

  • You can apply anytime, but remember we can only proccess your application with a max of 30 days prior arrival.

  • Passengers must have a medical certificate with a negative Coronavirus (COVID-19) PCR test result. The test must have been taken at most 96 hours before departure from the first embarkation point. The certificate must be in English. This does not apply to passengers younger than 10 years.

  • Passengers are subject to medical screening and self-isolation for 7 days upon arrival, and to a Coronavirus (COVID-19) PCR test within 7 days after arrival.

  • Country requirement at entry: a yellow fever vaccination certificate is required for all travellers aged 9 months or over.

  • Passengers departing from the United Kingdom and Republic of South Africa MUST show evidence of having paid for their post-arrival Day-7 COVID-19 PCR test prior to boarding.

All passengers arriving in Nigeria will be required to proceed with a mandatory self-isolation period of 7 days in their selected place of abode. Quarantine in the city of the port of entry is not necessary. Those travelers developing symptoms of COVID-19 on arrival in Nigeria or during screening will not be allowed to self-isolate and will be placed in institutional quarantine. Additionally, Nigerians and holders of a permanent residence permit without any of the following TWO documents (a negative COVID-19 PCR test result taken not more than 96 hours before boarding and the health declaration) are subjected to a mandatory quarantine of 8-14 days (depending on results of COVID-19 PCR test done from day 7 after arrival). All travelers who test positive for COVID-19 after the post-arrival day 7 test shall be managed based on national guidelines for COVID-19 treatment.
