APPLYING FOR THE JORDAN HEALTH DECLARATION FORM
Obtaining the Jordan Health Declaration Form with iVisa.com is an easy and user-friendly process, as follows:

Do I need a QR code to enter Jordan?
Yes. All visitors to Jordan are required to fill out the Health Declaration form and obtain the QR code. This QR code must be presented upon boarding the plane to Jordan and upon arrival.
